Inspector Notes
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 52 children directly supervised by 10 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 6 children?s records, 1 medication, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaging in center play activities, playing on the playground, listening to story time, and making crafts. Infant care and feeding was also observed. Positive interactions between children and staff were observed during this inspection. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations
3Evidence: Peeling paint was found on the wall below the window by the door exiting the classroom.
Evidence: Child #7 was administered a prescription medication by staff during the inspection without written authorization from the parent.
Evidence: Two bottles of prepared infant formula were observed in the refrigerator in the Infant B classroom that were not correctly labeled. One bottle was not labeled with the date. The second bottle was labeled "Extra" and was not labeled with either the infant's name or date.
