Inspector Notes

An unannounced joint complaint investigation was completed to investigate an allegation regarding behavior guidance, supervision, care, protection, and guidance on July 26, 2023. The investigation was conducted with the assistant director beginning at 2:40pm and ending at 4:00pm. Interviews were conducted and facility documentation was reviewed. The allegations of behavior guidance, supervision, care, protection, and guidance were unable to be substantiated due to insufficient corroborating evidence.

Evidence: The record for Staff #1, with a start date of 5/31/23, did not contain documentation of the results of a search of a central registry search at the time of inspection.
Plan of Correction: The center will obtain a central registry for the staff record that was missing the documentation.
