Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/9/21 and concluded on 4/12/21. The director was contacted by telephone to initiate the inspection. There were 28 children and 6 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records, 3 staff records and additional center documents submitted by the facility to ensure documentation was complete. A virtual walk through of the center was conducted to assess the space and materials used by the children. Medications and medication authorization forms were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. 1. Child # 1's first day of enrollment was 1/19/21.
2. A copy of the Child # 1's physical exam was not obtained within 30 days of enrollment.
Evidence:
1. Staff # 1 started employment on 3/1/21.
2. The center did not obtain a complete SDS form for Staff # 1 before the first day of employment.
Evidence:
1. Staff # 1 first day of employment was 3/1/21.
2. Staff # 1's VA central registry findings were not requested nor obtained within 30 days of employment.
Evidence:
1. Staff # 1's first day of employment was 3/1/21.
2. Fingerprint results from Field Print were not obtained for Staff # 1 before the first day of employment, and were not available for review.
Evidence:
1. Staff # 1's first day of employment was 3/1/21.
2. Staff # 1's record indicated that she resided in Maryland from 12/2016 through 2/2018. Staff # 1 also resided in North Carolina from 3/2011 through 12/2016.
3. Staff # 2's first day of employment was 1/25/21, and his records showed that he lived in North Carolina within the last five years.
4. The center did not obtain results of out of state sex offender checks for Staff # 1 and Staff # 2 prior to their first day of employment.
5. Staff # 3 stated that out of state central registry child abuse neglect requests were not made for Staff # 1 and Staff # 2 since they began employment.
