Inspector Notes

An unannounced monitoring inspection was initiated on 3/2/22 and concluded on 3/4/22. There were 20 children present with 6 staff supervising. The children were observed playing outdoors, playing with blocks in the classroom. There were no medications on site. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children's records and 2 staff records were reviewed. Other center documents related to required policies were also reviewed and found to be in compliance.

Based on observation, areas and equipment outside the center were not maintained in a safe and operable condition.
Evidence: A white plastic pipe attached to the water table on the playground was broken, posing an injury hazard to the children.
Plan of Correction: The pipe will be removed.
