Inspector Notes

An unannounced renewal inspection was conducted from 10:15 am to 12:30pm. There were 22 children in direct care of 6 staff members. The children were observed in outdoor play, home living and reading. There were no children on site with medications. 3 staff records, 3 children's records, drill records, injury reports and other center documents were reviewed to assess compliance. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-350-B-2
Based on observation and staff interview, the staff to children ratio for children 16 months to 24 months was not maintained.
Evidence:
1. Staff # 1, Staff # 2 and Staff # 3 had a total of 13 children on the playground. 5 of those 12 children were under the age of two years. The ratio for children 16 months to 24 months is 5 children to 1 staff member.
2. At approximately 10:47 am, there were 12 children on the playground with two staff members.
3. Staff # 1 had taken one child to use the bathroom, leaving Staff # 2 and Staff # 3 with 12 children and out of ratio.
4. Staff # 2 stated that Staff # 1 was gone for approximately one minute.
Plan of Correction: The staff returned to the group. Staff will be retrained on ratios.
Standard 8VAC20-780-500-B
Based on observation, there was not a foot operated diaper disposal system in the toddler room.
Evidence: The diaper trash can in the toddler room was not foot operated. It had a top lid with two flaps that would need to be pushed for anything to go through.
Plan of Correction: A foot operated trash can has already been ordered.

Based on records review and staff interview, children's injury reports were not complete.
Evidence:
1. An incident report from 8/14/23 was missing the future prevention plan. The time of incident and the time parents were notified was also documented incorrectly. The time of parent notification was before the time of incident.
2. Incident reports from 8/21/23 and 8/24/23 did not include the future prevention plans.
Plan of Correction: Staff will be retrained on injury reports and they will be checked for accuracy and completion.
