Inspector Notes

A monitoring inspection was conducted on 4/29/24 from 10:35 am to 12:00 pm. There were 21 children present, with 4 staff supervising. The children were observed playing outdoors on the playground, painting, playing in home living and eating lunch. The lunch menu included turkey and pasta in pasta sauce, cantaloupe, salad, cheese, green beans and milk. 3 children's records, 3 staff records, attendance, emergency drill records and injury reports were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-2
Based on records review and staff interview, the center did not obtain results of a fingerprint based criminal record check (CRC) for a staff member prior to the first day of employment.
Evidence:
1. Staff # 1 transferred from an out of state location to this center. Staff # 1's first day of employment at this location was 1/16/24.
2. Staff # 1 stated that she had gone for the fingerprinting process and had documentation of the appointment in October, but the results could not be found and were not available for review.
Plan of Correction: We will try to locate the fingerprint results and if not found, a new appointment will be made no later than 5-9-24. Once the results are obtained, the licensing inspector will be notified.
Standard 8VAC20-770-60-B
Based on records review and staff interview, the center did not obtain a complete sworn disclosure statement (SDS) prior to an employee's first day of employment.
Evidence:
1. Staff # 1 transferred from an out of state location to this center. Staff # 1's first day of employment at this location was 1/16/24.
2. Staff # 1 stated that she had not completed a SDS.
Plan of Correction: The SDS was completed.
Standard 8VAC20-770-60-C-2
Based on records review, the center did not obtain results of a Virginia central registry search for a new employee within 30 days of employment.
Evidence:
1. Staff # 1 transferred from an out of state location to this center. Staff # 1's first day of employment at this location was 1/16/24.
2. The CRS for Staff # 1 has not been obtained to this date.
Plan of Correction: The CRS request will be followed up on and if not found a new one will be submitted no later than 5-9-24. Once the results are obtained, the licensing inspector will be notified.
