During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on July 30, 2025 and completed on July 30, 2025. The on-site inspection began at 12:05 pm and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 52 children present with 15 staff. The inspector reviewed 7 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, observations and interviews.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day
of employment.

Staff #7, who had been employed for more than 1 year, did not have a central registry
search requested. Staff #7 was working with children.
Plan of Correction: Contacted VA DSS and they resent results of background check.
Standard 22.1-289.035-B-4
Child day center shall obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Center did not obtain out-of-state background checks for Staff #6. Staff #6 has been employed for 11 months and was working with the children during today's inspection.
Plan of Correction: On 7/31/2025 contacted NC and requested background check. Mailed request form on 8/01/2025 and they will mail results.
