Inspector Notes

An unannounced inspection was completed on 10/31/2023 from 9:45am to 10:55am in response to a planned change related to the legal entity operating the program. Any areas of noncompliance are documented in the violation notice. The provider was present with one assistant. There were nine children in care, ranging in age from 17-months to three-years-old, for a total of 21 points. Children were observed reading books, practicing flashcards with the assistant, doing a stamping craft while discussing the days of the week in Spanish, cleaning up, and going outside to play. Two new child records were reviewed as well as updates for long-term children. The provider record, the assistant?s record, one adult household member record, and one pet record were reviewed. The provider and assistant have current certification in CPR and First Aid. Two medications and authorization forms were reviewed and the provider and assistant have current certification in Medication Administration Training (MAT). The first aid kit, flashlight, weather-band radio, smoke detectors, carbon monoxide detector, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Based on review of two medications and authorization forms, the provider did not obtain written authorization from a parent to administer emergency medications to a child in care. Evidence: The medication authorization forms for Child B?s emergency medications did not contain a parent signature authorizing the provider to administer the medications to the child.
Plan of Correction: The form was signed by the parent the same day. Moving forward we will make sure parents sign the authorization form when the medication is dropped off.
