Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/29/2021 and concluded on 04/30/2021. The director was contacted by telephone to initiate the inspection. There were 10 children present with 1 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center did not have documentation of at least one staff member on duty who has obtained, within the last three years, instruction in performing the daily health observation of children.

Evidence:
1. Documentation of training received in Daily Health Observation for at least one staff member present was not available for review during inspection.
Plan of Correction: We will have additional staff trained in Daily Health Observation.

Based on record review, 3 of 3 records of injuries did not contain all required information.

Evidence:
1. An injury report dated 2/23/2021 did not contain documentation of the date the parent was notified and how the parent was notified.
2. An injury report dated 3/5/2021 did not contain documentation of how the parent was notified.
3. An injury report dated 4/27/2021 did not contain documentation of the date the parent was notified.
Plan of Correction: We will handwrite the missing required information on our copies that are already printed and look at updating our template. Missing information for the injury reports reviewed was corrected during the inspection.
