Inspector Notes

A complaint investigation was initiated on 09/27/2022 and ended on10/06/2022 regarding forbidden actions. Interviews were conducted, and records and documentation were reviewed. The validity of allegations in the complaint could not be substantiated due to a lack of preponderance of evidence. Violations not related to the complaint were identified. These areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-60-A
Based on interview and record review, children's records did not contain all required information.
Evidence:
1) On the date of inspection, Staff #2 stated that Child #1 did not attend the program anymore.
2) The record for Child #1 did not contain the last date of attendance.
Plan of Correction: The child's termination date has been recorded in our student information system and is also recorded in the student's paper file.

Based on interview and record review, staff's records did not contain all required information.
Evidence:
1) On the date of inspection, Staff #2 stated that Staff #1 was not currently employed by the center.
2) The record for Staff #1 did not contain the date of separation from employment.
Plan of Correction: The termination date was recorded following Paychex protocol in our online system. Full paper copies of employee files are only maintained for employees subject to review for state licensing which does not apply to the employee in question.
