Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 11:45am and 3:30pm There were 16 children directly supervised by 2 staff. The physical plant, 3 staff records, 3 children?s records, 2 Agent records, evacuation drills, and policies were inspected. Children were observed listening to a bible study. The children were actively engaged in the lesson. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Areas of non-compliance are identified in the Violation Notice. Standard 22.1-289.036-A
Based on record review, the center did not obtain required background checks every five years as required.

Evidence:
1. The fingerprint based criminal history record check results in the record for Agent #1 were dated 9/1/2023. The previous fingerprint checks were obtained on 8/6/2018, more than five years prior.
2. The most recent fingerprint based criminal history record check results in the record for Agent #2 were dated 8/6/2018.
Plan of Correction: Fingerprints for Agent #2 have been submitted. Once results are received they will be provided to the Department.
Standard 8VAC20-780-130-A
Based on record review, the center did not obtain documentation the child received the immunizations required by the State Board of Health before the child can attend the center for 2 of 3 children's records reviewed.

Evidence:
1. Child #1 began attending the center on 8/22/2023. The immunizations in the record for Child #1 were dated 8/31/2023.
2. Child #2 began attending the center on 8/22/2023. The immunizations in the record for Child #2 were dated 8/24/2023.
Plan of Correction: We will make sure no student starts without the immunization records.

Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Cleaning materials were observed in an unlocked closet in the preschool classroom.
Plan of Correction: We will keep the closet locked.

Based on record review, the center did not meet the proof of child identity and age requirements as required by ? 22.1-289.049 of the Code of Virginia for 1 of 3 children's records reviewed.

Evidence:
1.The record for child #2 did not contain the date the original proof of child identity was viewed, and the signature of the person who viewed the original proof of child identity documents.
Plan of Correction: we will review proof of age and identity requirements with staff.

Based on record review, 2 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #2 did not contain the telephone number for one parent's place of employment.
2. The record for Child #3 did not contain the telephone number for each parent's place of employment.
Plan of Correction: We will call the parents and make sure we have the contact information.
