Inspector Notes
An unannounced complaint investigation was initiated on 9/16/2021 and was concluded on 10/05/2021 in response to a complaint received by the licensing office on 09/16/2021 regarding supervision. Telephone and email correspondence was exchanged with referring agency on 9/16, 9/17, and 9/21. Staff interviews and record reviews were conducted relating to the allegations on 9/21/21. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". Violations
5Evidence:
1. On 9/8/21 child #1(age 3) was mistakenly put on the daycare van with their siblings and transported by the daycare staff to a local public school and left at the school for approximately 6 hours.
A. There was no staff with child #1 during this time to ensure their care and protection.
B. The staff at the center stated they did not realize the child was not supposed to be at the school until they were contacted by the parent.
2. On 9/14/21 Child #2(age 4, nonverbal autistic child) left the building unsupervised:
A. they were intercepted by a school crossing guard while crossing a main four lane road
B. staff #2 stated a man was observed knocking on the entrance to the center and asked "are you missing a child"
Evidence:
1. On 9/8/21 child #1(age 3) was put on the daycare van with their siblings and transported by the daycare staff to a local public school.
2. Daycare staff left child #1 at the public school without daycare staff for approximately 6 hours.
3. On 9/14/21 Child #2(age 4, nonverbal autistic child) left his assigned group, assigned to staff #1 and staff #2. Child #2 exited the building, walked across the parking lot, and then across a four lane main road, where he was stopped by a school crossing guard.
4. Child #2 was out of the sight and sound supervision of staff for approximately 20 minutes.
5. Staff #1, 2, & 3 confirmed that they did not not know where child #2 was during this time.
Evidence:
Staff confirmed that Child #1's name was not listed as a child to be transported.
Evidence:
1. On 9/8/21 child #1 was transported by the daycare staff in the van to a public school. There was no documentation of parental permission for child #1 to be transported in their record.
2. Staff confirmed that there was no transportation agreement for child #1.
Evidence:
1. Child #2 was missing for approximately 20 minutes on 9/14/21. The local police were contacted. The commissioners representative was not notified by the center.
2. Inspector was notified on 9/16/21 by a referring community agency.
