During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/24/2025. The on-site inspection began at 10:10am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and four staff. The inspector reviewed five children?s records and five staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 updated screening was due three months ago. The updated screenings had not been completed by 06/24/2025.
Plan of Correction: We will have the staff member to update the TB screening.

Repeat Violation

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. In Classroom #1, two chairs had cracks in the seat, creating a pinching hazard. Children were observed in the classroom during the inspection.
Plan of Correction: The chairs were removed from the center during the inspection.

The required ratio for children aged two-years old is one staff per eight children. Staff #1 was observed supervising 10 children (youngest child two-years old) from approximately 10:10am-10:20am.
Plan of Correction: Additional children had arrived at the center. This staff-to-child ratio requirement was corrected during the inspection.
