Inspector Notes

An unannounced monitoring inspection was conducted on Tuesday, January 23, 2024. The inspection began at 9:00am and concluded at 11:30am. The center director was present during the inspection. There were 35 children present with 7 staff supervising. Upon arrival, the children were engaged in a variety of activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center failed to ensure that the findings of the most recent inspection of the facility is posted on the premises of each facility.

Evidence:
The most recent inspection dated 7/10/2023 was not posted at the center. The inspection posted was dated 1/30/2023.

Standard 8VAC20-780-160-A
Based on record review and interview, the center did not ensure that documentation of a tuberculosis (TB) screening was submitted for each staff at the time of employment and prior to coming into contact with children.

Evidence:
1. The record for staff #1, did not contain documentation of a completed Tuberculosis (TB) test/screening. Staff #1's date of hire was 10/16/2023.
2. The record for staff #2, did not contain documentation of a tuberculosis (TB) test/screening. Staff # 2's date of hire was 9/5/2023.
3. The record for staff #4, did not contain documentation of a tuberculosis (TB) test/screening. Staff # 4's date of hire was 8/22/2022
4. The record for staff # 5, did not contain documentation of a tuberculosis (TB) test/screening. Staff # 5's date of hire was 11/21/2022.
5. The director stated that staff tuberculosis (TB) test were complete for all staff but the results are kept off site at the administrative office. The inspector reached out to the administrative office to obtain tuberculosis (TB) results for staff. Tuberculosis results were not received.

Based on review of staff records, the center did not obtain updated tuberculosis (TB)screening or testing every two years for one staff member.
Evidence:
1. The record for staff # 3 contained a tuberculosis (TB) test dated for 3/8/2021. The tuberculosis (TB) should have been updated on or before 3/8/2023.
