Inspector Notes

An unannounced renewal inspection was conducted along with Christina Jones on 6/5/24 from 9:30 a.m. to 11:00 a.m. with the provider. There were four children in care, ranging in age from seven months to six-years-old, for a total of 10 points. Children were observed playing with age-appropriate toys. Four child records, one staff record, and two household member records were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider has current certification in Medication Administration Training (MAT). The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Standard 8VAC20-800-180-A
Based on a review of records and interview with the provider, the provider did not obtain subsequent tuberculosis (TB) screening, every two years from the date of the initial screening.

Evidence: The last documented TB test for Staff #1 was dated 1/4/21, for Household Member #1 was dated 1/12/21, and for Household Member #2 was dated 10/14/21.
Plan of Correction: The provider received a new TB test, and all household members listed received new TB tests.
Standard 8VAC20-800-240-A
Based on observation, the provider did not ensure that areas and furnishings of the family day home, inside and outside were maintained in a safe condition.

Evidence: A carpet with curled edges and a foam mat that did not lie flat was observed in the play area, both posing trip hazards.
Plan of Correction: The provider removed small rug with curled edge, and replaced it with a new one. The foam mat was removed, and new sections added.
Standard 8VAC20-800-430-B
Based on observation and interview the provider did not ensure that pressure mounted gates were not used as protective barriers at stair openings.

Evidence: A pressure mounted gate was observed at the bottom of the stairway.
Plan of Correction: The provider purchased a new non-pressure mounted gate to block the entry to stairs.

Based on a review of documentation and interview, the provider did not ensure that the emergency plan was reviewed at least annually and updated as needed.

Evidence: The last annual review of the emergency plan was dated 10/14/2013.
Plan of Correction: Provider reviewed and updated emergency plan and will conduct regular annual reviews.
