Inspector Notes
An unannounced monitoring inspection was initiated on 12-4-2024 and completed on 12-11-2024, in response to a self-report received by the licensing office on 11-19-2024 relating to an injury requiring outside medical attention and areas of Staffing / Supervision and Special Care Provisions/Emergencies. This inspection included document review, tour of the facility, interviews and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
