The areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There were 6 large bolts protruding from the playground boarder.

A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone. The required phone numbers were not posted in 5 out of 5 classrooms (classrooms, A,B,C,D and E) observed.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information. The record for Child #2 and Child #3 did not include the employment location and phone number for one parent listed.

Staff records for each person shall include all required items. Staff #1 was missing documentation to demonstrate they possess the experience and education required by the job position. Staff #1 is designated as program director.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance sheet for classroom E was not accurate as it only listed 5 children in care and there were 12.
