Inspector Notes
An unannounced Monitoring Inspection took place on 06/01/2023 between the hours of approximately 3:30 p.m. and 5:00 p.m. An investigation into a self-reported injury was also initiated. There were 4 classrooms observed with a total of 18 children with 5 staff within the supervision guidelines. The children were observed waking up from nap, eating snack, playing outside and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, injury reports and emergency procedures was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence: Child #3?s (age: 16 months) most recent documentation of immunizations was dated 10/14/2022.
Evidence: Child #2 (start date: 01/03/2023) did not have documentation of a physical examination on file.
Evidence: Staff #4 stated that the center does not currently have written procedures for injury prevention.
Evidence: Staff #4 stated that the center does not currently have written procedures for prevention of shaken baby syndrome or abusive head trauma.
Evidence:
1. The provision for active supervision did not include circulating among children.
2. The center?s written playground safety procedures did not include a method of maintaining resilient surface.
Evidence: The most recent practice evacuation drill implemented by the center was dated 03/14/2023.
