Inspector Notes
An unannounced renewal Inspection took place on January 22, 2024 between the hours of approximately 10:40 a.m. and 11:40 a.m. There were 3 classrooms observed with a total of 13 children with 5 staff within the supervision guidelines. The children were observed playing with manipulatives, engaging in free play and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and attendance was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 5Evidence: Child #1?s (start date: 05/05/2022) record did not contain documentation of immunizations.
Evidence: Child #1?s (start date: 05/05/2022) record did not contain documentation of a physical examination.
Evidence:
1. Staff #2 was observed changing Child #2?s diaper. Once the diaper change was complete, Staff #2 returned Child #2 to the play area.
2. Child #2?s hands were not washed before being returned to the play area.
3. Staff #2 was then observed placing Child #3 on the changing table for a diaper change.
4. Staff #2 did not was their hands between changing Child #2 and Child #3.
Evidence:
1. Staff #2 was observed changing Child #2?s diaper. Once the diaper change was complete, Staff #2 returned Child #2 to the play area.
2. Staff #2 was then observed placing Child #3 on the changing table for a diaper change.
3. Staff #2 did not clean and sanitize the changing table in between changing Child #2 and Child #3.
Evidence: Child #1?s (start date:05/05/2022) record did not contain documentation of viewing proof of the child?s identity and age.
