During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 8, 2025. The on-site inspection began at 9:30 a.m. and ended at 10:40 a.m. The inspector reviewed compliance in the areas listed above. There were 25 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 16, 2025. A POC submitted after this date will not appear on the public website.

Hazardous substances shall be kept in a locked place.

There was a spray bottle containing a bleach solution on a cubby, accessible to children ages two years old.

There were three disinfectant sprays, two containers of disinfectant wipes, and one glass cleaner in an unlocked changing table, accessible to children ages two years old.
Plan of Correction: Talked to staff about putting away spray bottles/cleaners in locked changing cabinet after use. Put a sign on the diaper changing table to remind them to lock the changing table after cleaning or diapering.

Fans, when used, shall be out of reach of children.

There were two fans in use and within reach of children ages two years old.
Plan of Correction: Removed fans from the classroom. If we use fans, we will make sure, they are out of reach of children.

REPEAT VIOLATION

Each center shall maintain a separate record for each child enrolled which shall contain name, address and home phone number of each parent who has custody; name, address and phone number of two designated people to call in an emergency; and name of persons authorized to pick up the child.

Child #1's record did not contain name, address and home phone number of each parent who has custody; name, address and phone number of two designated people to call in an emergency; and name of persons authorized to pick up the child. Child #1 has been enrolled for three weeks.
Plan of Correction: Child #1 parent completed the record for child with name, address and phone number of each parent who has custody, name and address of two designated people to call in an emergency and the name of people authorized for pick up.

I will review all the child's records on the first day of enrollment to make sure that child's paperwork has information completed.
