During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 17, 2025. The on-site inspection began at 10:35 a.m. and ended at 11:30 a.m. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 caregivers; the point total was 31. The inspector reviewed 2 children?s records on-site. This inspection included document review, tour of the family day home, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 1, 2025. A POC submitted after this date will not appear on the public website.

All applicants for employment, employees, applicants to serve as volunteers, and volunteers shall submit to fingerprinting prior to employment or beginning to serve as a volunteer.

Caregiver #1 did not submit to fingerprinting. Caregiver #1 was working at the family day home at the time of inspection.
Plan of Correction: All future caregivers will be with proper documents prior to care. This person is no longer with the daycare and will not return.

The provider shall maintain a record for each caregiver.

Caregiver #1 was working at the family day home during the inspection. The provider did not have a record for Caregiver #1.

The provider shall obtain from each caregiver at the time of hire a current Report of Tuberculosis (TB) Screening form.

The provider did not obtain a Report of TB Screening form for Caregiver #1. Caregiver #1 was working at the time of inspection.

REPEAT VIOLATION

Potentially poisonous substances shall be inaccessible to children.

A package of stainless steel wipes, two containers of disinfectant wipes, one disinfectant spray, one air freshener and one paint sample (label states to keep out of reach of children) were in an unlocked cabinet, accessible to children.
Plan of Correction: The cabinet has been locked and no longer accessible to children.

REPEAT VIOLATION

All flammable materials shall be stored in an area inaccessible to children.

A lighter was stored in an unlocked cabinet, accessible to children.
Plan of Correction: The lighter had been moved to a different cabinet that is out of reach to the children and locked.

REPEAT VIOLATION

A child's record shall contain an address of two designated persons to contact in case of an emergency if the parent cannot be reached, proof of the child's age and identity and written authorization for emergency medical care should an emergency occur.

Child #1's record did not contain and address for one out of two emergency contacts written authorization for emergency medical care. Child #1 has been enrolled for one month.

Child #2's record did not contain proof of the child's age and identity. Child #2 has been enrolled for over 4 months.
Plan of Correction: All of the Childrens records have been updated.

Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.

Caregiver #2 did not ensure that Child #2's hands were washed after diapering.
Plan of Correction: The children and caregiver have been talked to and reminded to always wash hands.
Standard 8VAC20-800-690-D
The diapering surface shall be nonabsorbent.

The diapering surface was covered with a fabric changing pad cover.
Plan of Correction: Fabric was removed, only the non absorbent portion was left.

Documentation shall be maintained of emergency evacuation drills.

There was no documentation of emergency evacuation drills for November 2024, December 2024, January 2025 and February 2025.
