Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 05/04/2021. The Provider was contacted by telephone to initiate the inspection. There were (3) children present, (6) points, with (2) caregivers. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) caregivers records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (3) non-compliances with applicable standards or law and were documented on a Violation Notice issued to the FDH.

Standard 22VAC40-111-140-B-3
Based on review, 1 of 2 staff files indicated that the Provider does not have a current certification in cardiopulmonary resuscitation (CPR).

Evidence -
1. On the date of inspection (05/04/2021), the Provider did not have a current CPR certification available for review.
2. The date of the most recent CPR certification on file expired - 04/05/2020.
Plan of Correction: The Provider is in contact with a training to schedule this training.

Based on review, 1 of 2 staff files indicated that the Provider does not have a current certification in First Aid.

Evidence -
1. On the date of inspection (05/04/2021), the Provider did not have a current First Aid certification available for review.
2. The date of the most recent First Aid certification on file expired - 04/05/2020.
Plan of Correction: The Provider is in contact with a training to schedule this training.
Standard 22VAC40-111-90-C
Based on review, 1 of 2 children files indicated that the family day home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (05/04/2021), a current (within the past 6 months) immunization record for Child #2 was not available for review.
2. The date of the of the most current immunization page on site for Child #2 was dated ? 09/25/2020.
Plan of Correction: The parent of Child #2 shall be contacted in order to obtain a current immunization page.
