Inspector Notes
A renewal inspection was initiated on 9/8/2022 and concluded on 9/8/2022. There were a total of 7 children ( 16 points) observed with 2caregivers present. The children were engaged in the following: toy play, games, learning about shapes, coloring, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 3 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
8Evidence: There was no documentation of annual training for either caregiver for 2021.
Evidence: Electrical outlet near rolled up mats did not have a child-resistant cover installed.
Evidence: The gate at the bottom of the basement stairs was being held closed with a bungie cord instead of the original latch. The gate could be pulled open with little force and could snap the gate shut on a child.
Evidence: Emergency contact information for Child #2 and Child #3 did not contain names of people other than the custodial parents who are authorized to pick up the child.
Evidence: The diapering pad was torn, exposing absorbent foam.
Evidence: There was no annual written parental authorization for topical skin products to be used for Child #1.
Evidence: The first aid kit only contained 1 triangular bandage.
Evidence: The emergency response drill log did not contain documentation of shelter-in-place drills.
