Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 4/14/21 and concluded on 4/15/21. The provider was contacted by telephone to initiate the inspection. There were three children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of two household member records and interview with the provider, the provider did not obtain the results of a search of the central registry or of the sex offender registry for another state in which a household member lived in the previous five years as required. Evidence: Household member A disclosed on their sworn statement dated 10/31/19 that they lived in the State of Tennessee within the past five years, but there are background checks from Tennessee on record and the provider confirmed that the requests have not been completed.
Plan of Correction: I have attempted to reach out to get them but haven't reached anyone. I will submit the requests as soon as possible.
