Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated virtually and concluded on Thursday, August 5, 2021.The provider was contacted by telephone to initiate the inspection. There were 11 children present and 3 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver and 1 household member's records, along with any requested program records submitted by the facility to determine if required
documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of caregiver's record and interview, the provider did not ensure to obtain a current certification in cardipulmonary resuscita.

Evidence: In Caregiver's #1's record, the certification in cardiopulmonary resuscita was due for renewal on 10/2020. Interview with provider confirmed this was the most recent certification available for review during this inspection.
Plan of Correction: The provider will call to schedule training for first aid and cardiopulmonary resuscita today.

Based on review of caregiver's record and interview, the provider did not ensure to obtain a current certification in first aid.

Evidence: In Caregiver's #1's record, the first aid certificate was due for renewal on 10/2020. Interview with provider confirmed this was the most recent certification available for review during this inspection.
Plan of Correction: The provider will call to schedule training for first aid and cardiopulmonary resuscita today.
Standard 22VAC40-111-180-A
Based on review of caregiver's record and interview, the provider did not ensure to obtain a current Report of tuberculosis screening form, every two years from the of the first screen or more frequently as recommended by a physician or the local health department.

Evidence: In Caregiver#1's record, the last tuberculosis screening was dated 04/10/19. Interview with the provider confirmed this was the most recent screening.
Plan of Correction: The provider will schedule appointment for tuberculosis screening today and include results in record once received.
