Inspector Notes

A monitoring inspection was initiated and concluded on Monday, May 9, 2022 in person at the family day home by the inspector. There were 12 children totaling present, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of
2 child records and 1 caregiver, 1 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of record and interview, the provider did not obtain for each adult household member a current TB screening for every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence: In the household member's record, the last dated TB screen was 10/21/2019.
Plan of Correction: The caregiver will ensure the household member schedule and complete an update TB screening and place in record for review.
