Inspector Notes

The licensing inspector initiated an unannounced complaint inspection on March 16, 2022 in response to a complaint received by the licensing office regarding staffing and supervision and special care provision and emergencies. Review of center documentation, interviews with staff members and observations were conducted. Based on information gathered during the investigation, there was preponderance of evidence to support the allegation regarding special care provision and emergencies therefore the complaint is determined to be Valid. Area of non-compliance are noted in the Violation Notice.

Based on staff interview, the licensee did not ensure parent was notified by the end of the day of minor injury.

Evidence: On March 10, 2022 , child #1 bumped his head and parent was not notified. Staff #1, Staff #2 and Staff #3 confirmed they did not notify parent.
Plan of Correction: Ensure that it is reiterated to staff how to handle emergencies. Communication of injuries and all important information.
