Inspector Notes
An unannounced complaint investigation was conducted on 7/1/2022 regarding special care provision and emergency and staff qualifications and training. Interviews were conducted and records were reviewed. The preponderance of evidence support the allegations in regards toe special care provision and emergencies and the complaint has been deemed valid.

Violations
3Evidence:
Staff #1 confirmed she was not made aware of the injury until the next day. Staff #2 confirmed she did not notify parent of injury and did not access the child with sustaining a serious injury. Staff#3 confirmed he did not notify the parent of the injury by the end of the day and did provide ice packs throughout the day.
2. If an injury is deemed serious 911 must be called and the parent must be notified. An incident report will be filled out and signed by the reporting staff and program director on the same day the injury occurred. The parent must also sign the form indicating they have been made aware of the incident on the day of the injury.
3. If an injury is not deemed serious but the child is in pain or discomfort the parent must be notified. An incident report will be filled out and signed by the reporting staff and program director on the same day the injury occurred. The parent must also sign the form indicating they have been made aware of the incident on the day of the injury.
4. Incident reports will be filed and saved for future reference.
5. If a serious injury does occur and outside medical treatment is required, WISC will report the incident and forward the incident report to the Department of Education Child Care VA within 2 business days.
6. Injury and incident protocols will be communicated to all staff moving forward in person and via email.

Staff #1, Staff #2 and Staff #3 confirmed there was not an injury report completed on the day of occurrence.

Evidence: On June 22, 2022 Child #1 received outside medical treatment. The center was notified on June 23, 2022. The center did not report the injury to the Health and Safety office in two business days. Staff #1 confirmed on July 1, 2022 the report was not submitted.
