Inspector Notes
An unannounced monitoring was conducted on April 20, 2023. There were 95 children present upon arrival. Additional administrative staff were present. Children were observed transition outside, coloring, art activities, freely playing and writing activiity. There were 7 staff records and 3 children records reviewed . Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and sheltering-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence: The record for staff #2, (date of hire: 8/29/2022) who indicated they lived out of state in the past five years, did not contain documentation of an obtain a copy of the results of a criminal history record information check prior to employment and a search of the child abuse and neglect registry or equivalent registry with in 30 days from employment for the state of Wisconsin.
Evidence:
Staff #1 (date of hire 05/15/2022) did not have documentation of results of the central registry findings.
Evidence:
Staff #2 ( hire date 8-29-2022) did not have documentaiton of completeing Virginia Department of Education-sponsored orientation.
Evidence:
Confirmed with staff that they were unable to locate documentaiton of 2022 monthly emergency evacuation, twice a year shelter in place and once a year lockdown drills.
Evidence:
Child #1 , Child #2 and Child #3 were missing the agreement that states the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
