Inspector Notes

A renewal inspection was initiated and concluded on 11/28/2023.There were 92 children present with 14 staff supervising and 2 administrators. The children were observed during classroom time engaged in lessons and observed during mobile library time. A total of 8 child records and 5 staff records were reviewed. The inspection started at 10 am and concluded at 12:20 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on record review and staff interview the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: The director confirmed that Child #4 (start date 8/21/2023) did not have documentation of immunizations.
Plan of Correction: carefully review documents to confirm name on document is of student not sibling. We have reached out to family and got correct document.
Standard 8VAC20-780-140-A
Based on record review and staff interview the center did not ensure each child shall have a physical examination before the child?s attendance; or within 30 days after the first day of attendance.
Evidence: The director confirmed that Child #6 (start date 8/28/2023) did not have documentation of a physical examination.
Plan of Correction: Gathered final page of physical. In future make sure all pages are attached.
