Inspector Notes
An unannounced monitoring inspection was initiated on 7/8/2024 and completed on 7/16/2024 in response to a self-report received by the licensing office on 7/2/2024 relating to an injury requiring outside medical attention. The inspector reviewed 4 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Supervision of Children, First aid training and cardiopulmonary resuscitation, and background checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/23/2024. A POC submitted after this date will not appear on the public website
Standard 8VAC20-770-60-C-2
(Repeat Violation) Providers must obtain the results of a central registry search for a staff member within 30 days of service. Staff #3 provided care to the children beginning 3/11/2024 and Staff #4 provided care to the children beginning 3/1/2023. There were no central registry search results for either staff member during the 7/8/2024 inspection.
Plan of Correction: Central registry was contacted via email & phone. Program manager has email access to CRS results.
Standard 8VAC20-780-340-D
(Repeat Violation) In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. In 2 separate groupings of children the staff did not meet the qualifications for a program lead or program director. Staff #1 and Staff #2 were responsible for a group of 18, 8 & 9-year-old children, and Staff #3 and Staff #4 were responsible for a group of 17, 11 & 12-year old children.
Plan of Correction: Program manager now understands the qualifications and has given additional training requirements.
Standard 8VAC20-780-530-A
(Repeat Violation) At least one staff in each classroom or area where children are present shall have current certification in CPR and current certification in First Aid. Staff #3 and Staff #4 were alone with their group of 17, 11 & 12-year-old children. There was no evidence of the completion of
CPR or First Aid certifications.
Plan of Correction: CPR schedule for 7/22/24 and is mandatory to continue work.
