Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #3 TB was dated seventeen months prior to date of employment.
Plan of Correction: Going forward all TBs will be completed 30 days to start date.
