Inspector Notes

A focused monitoring inspection was initiated on August 18, 2023 from 1:35pm - 3:00pm and concluded on August 24, 2023.The inspector reviewed compliance in the areas of background checks, staffing and supervision, programs and special services. A total of 9 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review and staff interview, the licensee did not ensure a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #2 lived outside of Virginia in the past five years.
Staff #2 lived in Alaska. Staff #2 hire date was 05/22/2023.
Staff #1 and Staff #3 confirmed they were not able to provide documentation of Alaska Central Registry results.
Plan of Correction: A criminial history search was sent off for staff #2 for the timein which she lived in the state of Alaska.
Standard 8VAC20-780-340-F
Based on observation and staff interview, the licensee did not ensure children under 10 years of age is always within actual sight and sound supervision of staff, except staff need to only be able to hear a child who is using restroom.

Evidence:
During the tour of the facility, a 8 year old child left the gym and walked to the bathroom. Confirmed by Staff #1 that there was not a staff in actual sight and sound supervision of the 8 year old child.
Plan of Correction: Staff members will be positioned in program area so that all children are within sight and sound. Staff will communicate when students are going to restrooms so that a staff can monitor restrooms.
Standard 8VAC20-780-430-K
Based on observation and staff interview, the licensee did not ensure individual space for wach child's personal belongings.

Evidence:
In the teen room/ mulit purpose space , the children belongings were stack on top of each other in a pile. There was designated indvidual space in the room but it was not being used. Picture was taken and provided with the inspection documents.
Plan of Correction: Staff members will enforce the rules that each child belongings are placed in cubby boxes while in each space. The designated space will be used for storage of belongings.
Standard 8VAC20-780-560-M
Based on observation and staff interview, the licensee did not ensure children would not be drinking or eating while walking around.

Evidence:
There were multiple children outside walking around eating popsicles at the pavilion area and around the pavilion area.
Plan of Correction: During snack or lunch times children will remain seated while they are eating.
