Inspector Notes

A renewal unannounced mandates monitoring inspection was initiated on November 22, 2023 from 8:37 am - 10:24 am. There were 16 children with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed. The children were observed eating snack and participating in multiple station activities. The outside area was not inspected. The muti - purpose room and bathroom was inspected.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the licensee did not ensure staff had a central registry finding withins 30 days of employment or volunteer services.

Evidence:
Staff #3 hire date 09/26/2023. Staff #3 did not have documentation of the results with in 30 days of employment. The center documentation of follow up of results was 11/17/2023.
Plan of Correction: The center will ensure staff have a CRS finding with in 30 days of employment or volunteer.

Based on record review and staff interview, the licensee did not ensure fire inspections were completed annually.

Evidence:
The most recent fire inspection was 08/12/2022.
Plan of Correction: The center will call the Fire Marshall to schedule a fire inspection.
Standard 8VAC20-780-260-B
Based on record review and staff interview, the licensee did not ensure there was a health inspection completed annually.

Evidence:
The most recent health inspection was August 2022.
Plan of Correction: The center will call the local Health Department to schedule enviromental inspection.

Based on observation and staff interview, the licensee did not have sheter in place procedures or maps posted in a location conspicuous to staff and children.

Evidence:
There was not shelter in place procedure or map posted in conspicous area in facility on the day of the inspection.
Plan of Correction: Shelter in place procedures will be posted in each programatic space.
