Inspector Notes

An unannounced inspection was initiated on 07/03/2025 and completed on 07/16/2025 , in response to a complaint received by the licensing office on 07/01/2025 relating to allegations of special care provision and emergencies and administration. The inspector reviewed a child's records and staff record. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/23/2025. A POC submitted after this date will not appear on the public website.

The center shall notify the parent by the end of the day of any known minor injuries.

The center confirmed the parent of child #1 was not notified of injury by the end of the day.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

The injury report did not have all required components completed on the day of occurence.

Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

The child #1 received outside medical treatment due to injury occurred at the center on June 30, 2025. The center did not report the outside medical treatment to the department.
