Inspector Notes

An unannounced monitoring inspection was initiated on 08/05/2025 and completed on 10/09/2025 in response to a self-report received by the licensing office on 08/01/2025 relating to administration and staffing and supervision. The inspector reviewed 2 children?s records. This inspection included document review, tour of the facility, interviews, and observations .

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 15, 2025. A POC submitted after this date will not appear on the public website.

When staff supervise children, they always ensure their care, protection, and guidance.

On July 31, 2025, Child #1 (6 years old) and Child #2 (5 years) were being picked up . Child #1 and Child #2 left the pavilion area alone, and was found by staff #1 in a parents car with windows down in the parking lot.

Child #1 and Child #2 were not supervised by staff for approximately 20 minutes. Staff were not aware the children left the group.

The children were found in a car in the parking lot on the side where the construction work is being completed.

The temperature was around 94 degrees.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On July 31, 2025, Child #1 and child #2 were not within actual sight and sound supervision of staff for approximately 20 minutes.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The centers did not follow their policy for monitoring children's arrivals and departure. The policy stated "All children must be accounted for in the program at all times. " Child #1 and Child #2 were not accounted for in post camp.
