Inspector Notes
An unannounced monitoring inspection was initiated on 08/22/2025 and completed on 10/09/2025 in response to a self-report received by the licensing office on 08/20/2025 relating to special care provision and emergency and staffing and supervision. This inspection included document review , tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/15/2025. A POC submitted after this date will not appear on the public website.
Violations
2The center shall notify the parent by the end of the day of any known minor injuries.
The center did not notify the parent that child#1had a minor injury.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
There was not an injury report completed for child #1 on day of occurrence.
