22.1 Early Childhood Care and Education

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 6, 2025 and completed on March 11, 2025. The on-site inspection began at 11:03 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers; the point total was 20. The inspector reviewed 2 children?s records and 1 caregiver/household member(s) records on-site on March 6, 2025.

This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation and interviews, the provider did not ensure equipment and materials used by a child should be appropriate to the age, size, ability, and interest of the child. A 19 month old was placed in a highchair for lunch time. Provider acknowledged this child is self-feed by staff.

Staff shall wash hands with soap and running water before and after a diaper change. Staff #1 did not wash hands before or after changing a child?s diaper.

Standard 8VAC20-800-690-E
The provider should ensure soiled disposable diapers and wipes should be disposed of in a leak-proof or plastic lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Staff #1 touched three doorknobs after diaper changing three children. Staff #1 disposed all the diapers in a plastic bag which was on the floor.

Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for Child #2 not have a parent?s signature for an annual review of the emergency contact information. Child #2 had been in care for over a year.

Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2, 22 months old, last documented immunization was over 19 months ago.

The provider shoudl ensure high chairs, infant carrier seats, or feeding tables with safety waist and crotch straps fastened according to the manufacturer's instructions should be used for children under 12 months of age who were not held while being fed. Staff #1 placed a 19 month old in high chair for lunch.
