During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on February 10, 2026 and completed on February 17, 2026. The on-site inspection began at 8:50 am and ended at 9:30 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 13. The inspector reviewed 2 children?s records and 1 caregiver/household member(s) records on-site on February 10, 2026. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on February 23, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver. Caregiver #1 who has been caring for children for one year did not have completed reference verifications on file with date of contact, signature of person who completed the reference check, and the relationship of reference for Caregiver #1.
Plan of Correction: I will do reference for staff in a week.
Standard 8VAC20-800-200-A
The provider must orient assistants by the end of their first week of assuming job responsibilities. Assistant Caregiver #1 provided care for children for one year without receiving orientation.
Plan of Correction: I will do orientation for staff.
