Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 9/30/2021 and concluded on 10/4/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 8 children (20 points) present, ranging in ages from 11 months to 3 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 3 caregiver records, and 1 household member's record were reviewed.

Based on a review of caregiver records, 1 out of 3 caregivers did not have a central registry background check every five years.

Evidence:
1. Caregiver #2 (doh: 8/8/2016) had a central registry background check on file that was dated 9/13/2016.
Plan of Correction: I will send it out right away.
