Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 10/19/2022. At the time of inspectors' arrival there were 74 preschool aged children in care with 10 staff members. Children were observed interacting with staff, nap, and departure. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review it was determined that the center did not maintain and keep at the center a separate record containing all of the required information for each child enrolled.
Evidence:
The records for child #1, child #2, and child #3 did not contain proof of identity and age.
Plan of Correction: Staff will request this information from the parent.
