Inspector Notes

An unannounced, on-site renewal inspection was completed on 10/21/2025, as a part of the licensure period. The on-site inspection began at 1:00 pm and ended at 2:45 pm. The inspector reviewed compliance in the areas listed above. There were 81 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-450-A
Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Cribs when being used by infants shall have a bottom cover.

3 of 5 classrooms had some children napping without the required linens on their cots.
Plan of Correction: Center will ensure that all children have top and bottom covers for cots.
Standard 8VAC20-780-510-B
Prescription and nonprescription medication shall be given to a child only with written authorization from the parent.

Medication was administered to child #6 without written parental permission.
Plan of Correction: Center will request parents signature on medication administration authorization form.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.

The record for child #2 did not contain the two required contacts.
The record for child #3 did not contain one of the two required contacts.
Plan of Correction: Center will request information from parents.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #1 and child #3 were both diagnosed with food allergies and there were no written care plans for either child.
Plan of Correction: Center will requested allergy plans during inspection.
