During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 05/06/2025, as a part of the licensure period. The on-site inspection began at 10:39 AM and ended at 11:42 AM. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 5. The inspector reviewed 2 children?s records and 1 caregiver and 1 household records on-site.
This inspection included; document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Household member 1's subsequent screening result was overdue by 8 months.
Plan of Correction: They will get a new TB screening as soon as possible.
Standard 8VAC20-800-340-C
The bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap, and paper towels.
There were no paper towels available for the bathroom.
Plan of Correction: I will put paper towels near the bathroom.
Standard 8VAC20-800-760-A-1
A first aid kit that contains at a minimum; Adhesive bandages, assorted sizes.
There were no bandages of assorted sizes available in the first aid kit.
Plan of Correction: I will add bandages to the first aid kit.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
The annual review for Child #2's record was overdue by 6 weeks.
Plan of Correction: I will have the parent review and update the file tonight.
