During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 7/29/2025, as a part of licensure period. The on-site inspection began at 12:00 PM and ended at 1:10 PM. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 6. The inspector reviewed 3 children?s records and 5 caregiver/household members records on-site.
This inspection included; document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4by the Superintendent shall be posted in a conspicuous place on the licensed premises. The most
recent inspection paperwork was not posted. Documents posted were dated 1/2023.
Central Registry within 30 days of a 14-year old residing in the home.
The Central Registry Search was overdue by 8 months for Household Member #4.
The provider shall obtain for each adult household member a current Report of Tuberculosis
Screening form, every two years from the date of the first screening.
Household member #3's subsequent screening result was overdue by 3 months.
Annually, the provider shall review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed
acknowledgment of the review.
The annual review for Child #2's record was overdue by 16 months.
