Inspector Notes

An unannounced monitoring inspection was conducted on 5/8/24 from 9:15 a.m. to 10:40 a.m. with the Director. There were 28 children in care, ranging in age from three to five-years-old, supervised by six staff. The children were observed writing their names and engaged in circle time activities. Five child records and five staff records were reviewed. The Center has five staff with current certification in CPR and First Aid, and three staff trained in Daily Health Observation Training. Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 12/21/23 and the most recent Health Inspection on file was dated 2/15/23. Areas of non-compliance are identified in the Violation Notice. Standard 22.1-289.035-B-2
Based on review of five staff records, the program did not obtain the results of a national fingerprint-based criminal history record check for one staff prior to employment.

Evidence: The fingerprint results on record for Staff #4, with a start date of 11/3/23, were dated contained documentation of a national fingerprint results dated 11/6/23.
Plan of Correction: Will make sure to communicate "Start Date" not "Hire Date" to avoid confusion.
Standard 8VAC20-770-60-B
Based on review of five staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to employment.

Evidence: The record for Staff #4 with a start date of 11/3/23, the sworn statement on record was dated 11/14/23.
Plan of Correction: Will make sure there is no confusion between "Hire Date" and "Start Date".
Standard 8VAC20-780-160-A
Based on a review of five staff records, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children.

Evidence:
1. The record for Staff #4, with a start date of 11/3/23, contained documentation of a tuberculosis screening dated 11/16/23.
2. The record for Staff #5, with a start date of 8/3/23, contained documentation of a tuberculosis screening dated 8/7/23.
Plan of Correction: Staff had TB prior to coming in contact with children.
Standard 8VAC20-780-70
Based on a review of five staff files, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence:

1. The record for Staff # 4, with a start date of 11/3/23, contained documentation of two references dated 11/9/23.

2. The record for Staff #4, with a start date of 8/3/23, contained documentation of one reference dated 8/11/23.
