During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/11/2025. The on-site inspection began at 2:00pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. Earlier in the day, there were 39 children present and eight staff. The
inspector reviewed five children?s records and five staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #2
updated TB screening was due two months ago and Staff #4 updated TB screening was
due three months ago. The updated screenings have not yet been completed.
Plan of Correction: The information was obtained during the licensing inspection.
Standard 8VAC20-780-70
Staff records shall include an emergency contact to be notified in an emergency and information about any health problems that may interfere with fulfilling the job responsibilities. This information is required to be kept at the center. The record for Staff #1
did not include this information.
Plan of Correction: The staff members schedule an updated TB screening during the licensing inspector.
