Inspector Notes

A monitoring inspection was conducted on October 26, 2022 from 1:30 pm until 5:20 pm. There were one hundred forty-nine children present, with thirty-two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records, three medications and paperwork, and ten staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on review of ten staff files, program failed to ensure that the results of a central registry check were completed within 30 days of employment for one staff.

Evidence:
1. The first day of employment for Staff #5 was 9/1/2022. There were no results in the file, but a copy of the request that was notarized on 8/26/2022.
2. The Operations Manager followed up with HR and they stated they had not received the request. The request was resubmitted to HR on 10/26/2022 for entry in the portal.
