Inspector Notes

An unannounced monitoring inspection was initiated on 03/15/2024 and concluded on 04/19/2024 in response to a self-reported incident received on 03/08/2024 regarding programs including forbidden action and behavioral guidance. Staff interviews were conducted. Documentation was reviewed. Video footage was observed.

One child's record and one staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Please complete the "plan of correction" and "date to be corrected" for the violation cited on the violation notice and return it within five (5) calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s)

Based on staff interviews, review of video footage and documentation, staff of the center failed to ensure behavioral guidance was constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence: On March 7, 2024, Staff #2 snatched a foam block from Child #1, three years old, and threw it over the playground fence. Child #1 retrieved another foam block, which Staff #1 attempted to take away. Child #1, holding onto the block, laid on the ground as Staff #1 wrestled the block from Child #1?s hand. Staff #1 looked at Staff #2 and loudly stated ?he needs his ass beat? and threw the block over the fence.
