Inspector Notes
An unannounced monitoring inspection was initiated on 09/12/2025 and completed on 09/29/2025 in response to a self-report received by the licensing office on 09/04/2025 relating to Programs: forbidden actions and behavioral guidance. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, and interviews. An additional interview was conducted on-site on 09/29/2025.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
