Inspector Notes
An unannounced monitoring inspection was conducted on May 20, 2024 from approximately 4:30 pm to 5:25 pm. Two staff were present with seventeen school age children. Children were observed participating in various table activities in the cafeteria. Five staff and two child records were reviewed. Violation found is noted on the violation notice.

Based on review, facility did not ensure 3 of 5 child records have documentation of required information.
Evidence: The following was found during record review:
1. The records for child #2 (start date 9/13/2022), child #4 (start date 6/19/2023) and child #5 (start date 8/21/2023) did not have documentation of parent's employment phone number.
2. The record for child #5 did not have documentation of physician information.
